Here  is  my  review.</p>

Sheehy<br />This  book  is  a  God-send  for  sinus  suffers.  The  regime  it  recommends  works  as  advertised.  I  may  attest  to  this&#8211;I  have  suffered  from  sinus  difficultnesses  all  my  life.The  5  step  solution  commended  by  Dr.  Josephson  REALLY  works.  Sinus  irrigation  for  example  has  made  a  huge  divergence  in  the  quality  of  my  life.  The  power  of  this  book  is  that  is  does  not  over-simplify.  Sinus  troubles  originate  from  a  range  of  complex  difficulties  and  these  need  to  be  understood.  What  this  book  does  well  is  break  down  the  solution  into  pieces  the  intermediate  person  may  comprehend  and  implement.<br />Barry  Sheehy</p>

Kahn<br />This  book  is  approachable  by  a  layperson  and  which  covers  most  of  what  a  sinus  sufferer  must  know  before  seeking  treatment.    It  provides  a  good  overview  of  the  anatomy,  a  brief  description  of  surgical  evaluation  and  intervention,  and  elaborated  descriptions  of  medications  and  commended  life style  adjustments</p>
<p>I&#8217;m  a  patient  of  Dr.  Josephson  and  he  strongly  inspires  people who are in need of medical care  to  read  this  book  so  I  read  it.    Before  reading  my  review,  I  commend  you  original  read  the  best  review  I  could  find,  written  by  Walt  Ballenberger  who  is  founder  of  PostNasalDrip,  &#8220;a  resource  web  internetlocation  for  sinusitis  sufferers  like  himself&#8221;.    You  may  find  the  review  at  the  following  links:  [..]    Ballenberger&#8217;s  review  identified  a  number  of  questions  which  were  unanswered  in  his  book.    By  talking  with  Josephson  in  person,  I  was  competent  to  get  answers  for  a good deal of  of  them.</p>
<p>LASER<br />The  book  doesn&#8217;t  explain  the  tradeoffs  of  laser  vs.  traditionalisti  cutting  instruments.    Before  Josephson,  I  saw  an  ENT  who  commended  laser  to  lightly  cauterize  the  turbinates  in  order  to  reduce  their  tendency  to  outstanding  when  inflamed  (the  scarification  reduces  the  membrane&#8217;s  elasticity).    Dr    Josephson  is  not  a  proponent  of  laser  except  in  sure  cases.    Although  a  laser  is  self-cauterizing  and  accordingly  reduces  bleeding,  it  causes  heat  harm  to  surrounding  tissue  (more  than  the  harm  caused  by  a  cutting  instrument)  and  instrument  setup  adds  time/cost  to  the  procedure.    Only  where  the  decrease  in  bleeding  outweighs  these  costs  is  use  of  a  laser  justified  (e.g.  when  the  patient  has  galore  little  polyps).    I  didn&#8217;t  ask  him  but  I&#8217;d  imagine  that  the  thermal  heat  injury  would  likely  cause  more  harm  to  mucus-producing  goblet  cells  and  mucus-transporting  cilia  cells  than  cutting  tools.</p>
<p>SINUPLASTY<br />This  book  doesn&#8217;t  explain  the  tradeoffs  of  sinuplasty&reg;  vs.  conventional  cutting  instruments.    Sinuplasty&reg;  is  a  proprietary  tool  made  by  Acclarent  which  uses  a  thin  balloon  similar  to  an  angioplasty  balloon  to  open  the  sinus  passages.  As  in  heart  vessels,  the  balloon  is  placed  into  position  and  inflated,  then  deflated  and  removed.  According  to  Acclarent&#8217;s  description,  this  results  in  permanent  widening  of  the  critical  areas  leading  into  the  sinuses  while  leaving  the  nose  lining  unharmed  rather  than  destructing  this  lining  as  occurs  in  typical  sinus  surgery.    I  would  have  to  imagine  that  if  the  pressure  applied  elaborates  bone  than  it  surely  will  crush  the  membranes  and  will have to  do  a great deal of  harm  to  them  and  the  cilia  which  are  so  necessary  to  remove  the  infection  and  mucus  (mucociliary  clearance)  which  is  the  whole  basis  by  which  the  sinuses  protect  you.  When  I  asked    Dr.  Josephson  with regards to  what  the  pressure  does  to  the  membranes  he  said  that  he  knew  of  no  studies  that  showed  what  the  effect  of  the  balloons  on  the  membranes  and  cilia  were.    It  turns  out  that  Dr.  Josephson  is  trained  in  sinuplasty  but  relies  closely  totally  on  FESS  (endoscopic  cutting  instruments,  as  described  in  his  book).    He  pointed  out  that  not similar to  angioplasty,  which  operates  on  only  soft  tissue  and  requires  a  stent  to  hold  open  the  expanded  tissue,  sinuplasty  elaborates  a  passageway  or  sinus  by  dilating  bony  openings  and  pathways&#8211;widening  one  passageway  by  crushing  an  adjacent  sinus  cavity.    He  speculates  that  such  pressure  fractured  cells  could  result  in  a  problem  later  such  as  mucocele  formation&#8211;the  routine  was  too  lately  formulated  and  long  term  studies  were  not  available.    In  addition,  while  sinuplasty  may  open  up  a  closed  frontal,  maxillary,  or  sphenoid  sinus,  it  cannot  open  up  the  labyrinth  of  the  ethmoid  sinus.    If  the  ethmoids  are  closed,  functional  FESS  may  be  required  in  conjunction.</p>
<p>FUNCTIONAL  ENDOSCOPIC  SINUS  SURGERY  (FESS)<br />Josephson  doesn&#8217;t  much  describe  his  surgical  proficiencies  in  this  book,  which  makes  it  difficult  to  compare  FESS  with  substitute  approaches.    Dr  Josephson  says  Functional  Endoscopic  Sinus  Surgery  (FESS)  uses  microcutting  instruments  to  remove  bony  partitions  and  to  widen  openings  into  the  frontal,  maxillary  and  sphenoid  sinus  when  necessary.  I  asked  how  he  elaborates  the  passages  to  closed  sinus  cavities.    He  explained  that,  in  my  case,  he  would  remove  the  wall  amid  the  passage  to  a  frontal  sinus  and  an  adjacent  ethmoid  sinus  cell  (rather  than  crushing  the  ethmoid  cell  as  in  sinuplasty).    This  would  be  more  controlled  than  expanding  a  balloon  (as  in  sinuplasty)  and  keep out of the way of  creating  a  new  closed  cell  which  could  become  a  cyst.    While  FESS  changes  the  connectivity  of  the  cavities,  it  wards off  potentially  closing  off  the  adjacent  cells.    He  claims  that  exploration  shows  that  mucus  transport  from  the  now-opened  frontal  sinus  to  the  ostiomeatal  complex  is  preserved  with  this  method.    This  book  must  include  and  exaggerate  on  these  descriptions.    I  also  asked  in  what  cases  the  membranes  grow  back  which  seems  indispensable  when  polyps  are  got rid of  from  sinus  cells,  leaving  bare  bone  walls.    He  replied  that  they  speedily  grow  back.    This  book  must  include  pointers  to  exploration  validating  this  claim.</p>
<p>SINUS  ANATOMY<br />This  book  describes  the  sinuses  as  cavities  which  clean  and  humidify  inhaled  air.  However,  the  diagrams  show  the  sinuses  as  dead-end  spaces  off  the  primary  airway  connected  only  by  tiny  passages.    Why  does  air  travel  through  them  except  a  little  amt  thru  turbulence?    Although  I  suspect  they  do  without doubt  clean  and  humidify  inhaled  air,  it  would  be  helpful  to  me  if  the  book  resolved  this  seeming  contradiction.</p>
<p>CAT  SCANS<br />The  book  includes  a  series  of  diagrams  showing  an  idealized  representation  of  the  sinuses  and  other  organs.    I  found  it  very  difficult  to  relate  my  CT  scans  to  these  idealized  diagrams  even  even though  I&#8217;m  rather  good  at  3D  visualization.    It  would  be  helpful  if  this  book  included  a  link  to  an  online  series  of  example  CAT  scans  with  annotation  explaining  what&#8217;s  what,  how  the  structures  relate  to  impairment of normal physiological function  symptoms,  and  (assuming  galore  patents  get  post-operative  CT  scans)  what  the  structures  look  like  after  surgery.    Josephson  told  me  that  he  had  a  CT  review  in  a  draft  of  the  book  and  Penguin  Publishers  necessitated  to  make  the  book  shorter  and  insisted  that  he  cut  it  out.  Eventually  he  plans  to  put  it  on  line.</p>
<p>Overall,  an  magnificent  book  and  highly  recommended.</p>

Whether  it is  your  hardware,  your  software,  spyware,  configuration  of  your  house,  your  ISP,  what  type  of  cordless  phone  you  use,  or  a  myriad  of  other  things,  your  experience  may  be  wholly  dissimilar  from  your  best  friend&#8217;s.    So,  this  isn&#8217;t  so  much  a  review  of  the  router  as  it  is  a  review  of  my  experience  with  it.</p>
<p>The  Good</p>
<p>This  router  has  all  of  the  bells  and  whistles  with  the  exception  of  dual  band  broadcasting.    QoS,  WISH,  VPN,  Port  Forwarding,  all  levels  of  security,  etc.    It  has  Gigabit  wired  ports.    You  can not  get  anything  more  elsewhere  without  doubling  the  price.    It  is  all  packaged  within  an  beautiful  set  up  that  provides  hope  that  this  will  be  your  last  router  for  rather  a heap of  time.    Set  up  is  easy  and  straight  forward.    And,  most  importantly,  you  in a literal sense  can not  get  a  rapidly and without delay  router  when  within  30  feet.</p>
<p>The  Bad<br />I  actually  wanted  to  keep  this  router  but  I  couldn&#8217;t.    My  main  problem  was  heat.    After  a  few  hours  of  use  (just  being  on  at  all),  this  router  was  almost  too  hot  to  touch.    I  in truth  counterchanged  it  figuring  it  must  be  a  bad  router  but  the  new  one  was  the  same  way.    And,  more  importantly,  when  it  got  hot,  I  got  intermittent  lag  by way of  my  wire  machine  on  my  internet  traffic.    Games  that  played  flawlessly  on  my  old  Buffalo  router  started  lagging;  video  started  stuttering;  downloads  started  pausing.    Diagnosed  it  with  my  ISP  as  being  on  my  side  of  their  modem.    When  my  old  router  was  plugged  in,  all  difficulties  went  away.    D-Link  tried  to  support  me  but  there  was  a  language  barrier  and  no  matter  what  we  did  with  QoS  and  WISH,  it  didn&#8217;t  go  away.    I  had  to  return  the  router.</p>
<p>The  Ugly<br />FYI:  Blue  lights  are  pretty  but  REALLY  bright.    Seriously,  you  could  read  by  them.</p>
<p>Like  I  said,  I  in truth  wanted  to  keep  it  but  I  had  to  replace  it  instead.    Your  experience  may  vary  so  I  would  give hope or courage to  you  to  undertake  it  because  it  is  a  very  feature-rich  router.    Just  keep  your  old  one  handy  just  in  case.</p>

<p>I  applied  to  have  a  few  difficulties  with  WiFi.  The  introductory  problem  was  plainly  the  range&#8230;as  I  said  a  few  moments  ago,  the  DIR-655  fixed  that&#8230;I  mean,  even  even though  it  claims  6x  range,  but  I  was  going  to  be  happy  with  just  a  25%  improvement&#8230;I  seriously  think  that  this  router  increased  my  range  50-75%!</p>

<p>I  want  to  mention  that  I  have  been  looking  at  draft-N  routers  for  a  few  weeks  now  and  have  been  reading  the  reviews.  The  reason  that  I  am  writing  this  review  is  that  it  was  so  hard  for  me  to  find  a  good  review  on  this  router.  Cnet  does not  even  have  an  official  review  (as  of  2/13/07)  but  it  does  have  very  positive  user  comments.  From  my  homework  though,  (user  reviews,  and  more  significantly  official  reviews  from  Cnet,  engadget,  etc)  each  Draft-N  wireless  router  has  galore  kind  of  problem.  Some  do not  work  well  in  environments  with  more  than  a  couple  neighboring  networks,  numerous  do not  have  good  throughput,  and  a great deal of  do not  have  better  range.  Again,  I  didn&#8217;t  want  to  buy  this  one  because  I  couldn&#8217;t  find  any  solid  review  on  it.  After  going  through  a  couple  of  dissimilar  Draft-N  routers,  Let  me  help  you  out.  This  is  the  one.</p>
<p>BTW&#8230;im  not  running  an  &#8220;N&#8221;  wireless  adapter&#8230;this  thing  even  increments  the  range  of  &#8220;B&#8221;  and  &#8220;G&#8221;  signals.  GREAT  JOB  D-LINK!</p>

Smaller  than  expected.  Fast  sufficient  to  play  HD-DVDs  wirelessly  to  my  notebook  (Dell  Inspiron  E1505,  802.11BGN,  945GM,  Core  Duo  (2)  2GHz,  Vista  Premium,  Arcsoft  Digital  Theater,  D-Link  DNS-323  NAS).  Great  speed  and  coverage.  Also  handles  HDTV  utterly  thru  HDHomeRun  device.</p>
<p>Don&#8217;t  forget  to  upgrade  firmware.</p>
<p>Update  11-Nov-2007:  This  router  may  be  my  greatest  gadget  surprise  this  year.  It  proceeds  to  carry out  better  than  expected,  totally  solving  all  past  wireless  issues  and  handling  all  tasks  such  as  streaming  HD-DVD.</p>
<p>Update  7-Apr-2011:  Another  reviewer  reports  that  the  device&#8217;s  adapter  is  no  longer  dual  voltage.  References  to  voltage  have  been  removed?  Interested  in  a  versatile  travel  router?  I  commend  TRENDnet  300Mbps  Wireless-N  Travel  Router  Kit  TEW-654TR  (Black).  It&#8217;s  wireless,  outstandingly  small,  100-240  volts,  has  an  Ethernet  port.</p>
